PROGRAM AND COACH EVALUATION FORM


    This Program Evaluation form may be used by families of LMSC players to submit opinions and observations about the program that their child played in during the past season as well as offer opinions and observations about the coaches that worked with their child during the season. Comments received will be held in complete confidence by the LMSC Officers. The coaches will NOT see the evaluations. The evaluations will be used to help us to attempt to make our programs and teams run smoother and more efficiently in future seasons.

    The person(s) submitting their evaluation MUST include their name and e-mail address. Anonymous evaluations will be discarded.
