EPYSA RISK MANAGEMENT REGISTRATION
Coaches of LMSC travel teams registering with EPYSA need to complete this. For the fall of 2007, this applies to all girls teams, as well as the following boys teams: LMSC Barcelona, LMSC Blue Devils and LMSC Mustangs.

On the first page of the form, it will ask you to select your status for your registration. There are 4 choices. For the most part, all of you will either choose one of the first 2 choices, which are:

  • For returning coaches: "I registered in 2006-07. This is my first registration in 2007-08"
  • For new coaches: "I have never registered before. I am new to EPYSA"

In Step 2 of the Contact Information portion, you can skip item II: "Parent Information for Minors"

As soon as you complete all steps of the form and submit it, please send club registrar Nancy Worby the confirmation email you will receive so she can continue with the database work for your players. Remember, all coaches must fill out a form to be included on the team roster and to get a coaches pass. She is at misl@comcast.net

Head coaches, assistant coaches and team trainers must all fill out this Risk Management form.  



US CLUB RISK MANAGEMENT REGISTRATION

Coaches of all teams registered with US Club Soccer must complete this risk management forms. This includes coaches of all intramural teams as well as boys travel team coaches (except for LMSC Barcelona, LMSC Blue Devils and LMSC Mustangs). Girls travel teams do not fill out this form.

Head coaches, assistant coaches and trainers must all fill out this form.

  Select your registration type: Coach, Assistant Coach or Team Manager (Trainers should select Coach or Assistant Coach)

  You only need to fill in the fields with the red asterisks: Don't forget to check the Yes or No button for US Citizen.  

  Read the Authorization & Liability Release: Select "I Agree"

PERMISSION TO TRAVEL FORM
This is a link on the EPYSA web page. The online form must be completed at least 30 days before your team plays in an out of state tournament. This applies ONLY to our teams registered as EPYSA and is not applicable to teams registered with US Club.


LMSC TRAVEL TEAM COACHES REIMBURSEMENT FORM
Reimbursement Request form for our Travel Team coaches (requires Adobe Acrobat Reader). Please note that this form is for the 2007-8 travel team season.


LMSC TRAVEL TEAM MEDICAL RELEASE FORM
Release / Authorization Form required of all travel team players. This form MUST be notarized and must be notarized within the last 365 days. Coaches are required to carry this form for ALL players to all games, practices and tournaments. (requires Adobe Acrobat Reader)


LMSC TRAVEL TEAM UNIFORM ORDER SUMMARY
For LMSC Travel Team coaches to bring to registration night and hand in. This is the form used to order jerseys at registration night and record how much money (registration fees, uniform fees, etc.) were paid that night by each player. (requires Adobe Acrobat Reader)


COACHES LIST OF REGISTRATION PROCEDURES BEFORE, DURING AND AFTER REGISTRATION NIGHT
This letter is for head coach and explains, step by step, how to go about registration procedures for their team and their players for the 2007 / 2008 season.
