    Welcome to the LMSC Travel Team Online Registration Program. Please follow the directions provided on each page. The directions are summarized below:

    1. Fill out all of the information below.
    2. When you are finished, please press the "All Information Is Entered" button at the bottom.
    3. Your registration form will appear in your web browser.
    4. Print the registration form.
    5. Press the "Create Release / Authorization Form" button.
    6. Print the Release / Authorization Form. Note: Be sure to sign the Release / Authorization form, then have it notarized!!!
    7. Press the "Fee Summary Form" button.
    8. Print the Fee Summary Form.
